Complete the form and, after Rotary International Plate, type in the name of your club in the space provided. Print the completed form (you only need the first page of the 3 page form), or print the blank form and complete it by hand.

Now you can mail the completed form and check to DMV per their instructions and ours. Or take the form and your checkbook to any DMV office and walk out with your Rotary tag.
